Help needed to customize form.

I am using a Loan Application Template that belongs to your company.  I am trying to format the template for my needs. I have 3 questions for the Business name can I combine both business name and Business address and how do I do this?  Also how can I make the text box bigger for the business Name?  I would like this as big as the address text box. Final question for some reason it looks like me form starts with page 2 not sure what happened to page 1 and how do I go about fixing this to start back at one?

I am not sure if I have understood your requirement correctly. Do you mean to say that you want to place business name and business address fields to e displayed next to each other?

You can change the "Size" attribute of textbox field to change the width of textbox. Please check the screenshot below:
