All My Forms are disabled . I thought I fixed the issue.. please help !

    Hi my forms have been disabled. I'm sure you can see that this is a reoccurring  problem  and that I am not phishing and upholding the terms and conditions of jot form.  Last time this happened, the rep who helped me looked at my forms and said although he sees I am not phishing , due to the automation of the system , some of the "words" contained in my forms may suggest so . Since then I have changed those forms and have not had a problem ... until now ? Is there any way to see what triggered the disable this time , so I can change it ? I am about to go live on my site, and wish to avoid this problem at all costs . Thank you for your help , you guys have been very accommodating . 

     

    Just a thought ...

    is it possible the word "key" is a trigger term ? I ask because I have not changed any words in my forms since my last disable which seemed to fix the problem , EXCEPT for either adding the word key (as you can see in my forms ) or creating a space between what was previously "keyword" and now may read " Key Word" ( for example) .... is that possibly the trigger?
