You may want to check first this guide Which Form Embed Should I Use on different web sites. If the particular embed method you have chosen does not work, please try the iframe method : How to get your Form IFrame Code
Hope this help. If you need further assistance on this, please inform us.

Hi Guys, thanks for the Iframe tip as I now have the form appearing on the page. I still have a difficulty in the the fields, for example name fields are now misaligned. I have tried adjusting the px etc but this doesn't alter the form layout alignment.
I am using an apple mac with MAC OS X version 10.5.8 and KompoZer version 0.8b3 for the web page design.
I am an absolute rank amatuer at this guys so any advice etc is truly very much appreciated.

Can you please provide us the URL where your form was used? We can check it further.
I initially tested your form on Mac OS X Safari 5.1 and everything looks fine though.
- FitchLegalGroupAnswered on April 01, 2012 at 06:42 AMHi Jonathon,
I am at this stage developing the site. I haven't as yet published to our
registered domain. I am effectively looking at it within the development
I now have a much fuller appreciation of the work web publishers do, and
consequently I am fast concluding I might be far better off sticking to law
- far less complex and intricate LOL.
Given your response I am now thinking it is perhaps field parameters set by
KompoZer in the page template style sheet maybe. It logically has to be my
end if all is checking out your end.
I will read some more.
Thanks so much for your help - absolutely amazing client service guys.

I agree very much with your assessment Paul.
Somehow perhaps Komposer have to do with the display issue. I don't have Komposer though, but when I check your form for CSS I did not find any custom inputs so when I tested it directly on the browser everything looks ok.
Thank your for the good feedback. Should you require further assistance later on, don't hesitate to contact us.
