Getting the Form disabled message

I see that your account is auto-suspended by our anti-phishing system which is strict to scan each JotForm to calculate the probability of phishing activities. I would also like to suggest you, avoid asking asking "Username", "Password" or "Credit card" related information on Jotform. As anti-phishing system consider those things as phishing activity, and automatically suspend the account.
