I see that your account is auto-suspended by our anti-phishing system which is strict to scan each JotForm to calculate the probability of phishing activities. I have manually checked your JotForms and they seems to be normal and clean. I have activated your account and you should be able to see your JotForms now. Sorry for the trouble caused.
I would also like to suggest you, avoid asking asking "Username", "Password" or "Credit card" related information on Jotform. As anti-phishing system consider those things as phishing activity, and automatically suspend the account.
