    Jotform not working on home page and stand alone pages

    I have a jotform button on the left hand side of my blogger blog that says "REQUEST" but I found out that the button only works on individual posts and does not work on the homepage neither does it work on stand alone pages of the blog. How do I get it to work on all pages?

     

    I installed it by pasting

    The JotForm code is conflicting with javascript already on your webpage. 

    You may be able to find a compatible lightbox script on the internet and render the form in an iframe within that.
