My account was disabled for "phishing," but I work for Miller Travel Company and the forms were all for Miller Travel Company and our website that is going live today. The website administrator tried to log on and it disabled our account, he was putting the forms on our website for us. I need to talk to someone ASAP about this. I am very unimpressed there is no contact number!!! What kind of company is this!?

We do not allow for the collection of credit card details directly through the form.  Your "One Time Payment Authorization" form requested such details and resulted in our phishing detector suspending the account.  As per our terms of use:

Collecting Sensitive Information. You may not use the JotForm to collect certain types of sensitive information, including but not limited to credit card information and any type of login credentials. You may collect some sensitive information such as social security numbers or driver’s license numbers, but you are required to use best security practices of JotForm including SSL and Encrypted Forms features. You are solely responsible for compliance with any data protection and privacy laws and rules applicable to the sensitive information.

I removed the credit card fields from your form and activated your account.  Your forms should be usable once again.
