I have attached 3 screen shots. I have attempted to add several fields to Kids Klub Pediatric Dentistry's form. I have even cloned the old one and created a new one in an attempt to make it publish correctly. I attached 3 screen shots to assist you in understanding what I need. Please call me at 7068185299

Unfortunately we do not have phone support available, but we will do our best to help you here on forum.
I have cloned the form you provided on my account and when I inspected my form I noticed that there are some fields that are shown inside of the Form Builder, but they are not shown on live form.
This is happening because those fields are hidden with condition that you have set inside of your form.
So if option "New Patient" for patient Type field is selected all fields on your form will be shown.
I also noticed that you have three other conditions for fields that are deleted from the form.
I would suggest you to delete or disable those conditions, or set them for the fields that exist on your form.
