move form into a document for email

    I'm trying to get my forms into a document form so I can send them in docusign to add into email. I am using Microsoft exchange. Ideas? Suggestions?

    I am assuming you want to embed your form to a word document. But unfortunately it is not possible to embed your form to a word document.  It is possible to have a fillable PDF copy of your form:

    http://www.jotform.com/fillable-pdf-form-creator/

    This is usable in a similar manner to a word document.
