After it errors, and we go back and select 'other' instead, the form submits with no problem.

I have checked the form and after investigating I found that the "Date Picker" widget that you have in your form is causing this issue, the one about "Incomplete Values".
This is how the issue comes, first the field "Mode of Transportation" is required as well as the Date Picker widget:
Then when the form is submitted, although the field "Mode of Transportation" is filled, it shows as required:
But if you set the "Date Picker" widget to un-required, then the issue is not there.
I will forward this thread as I have set all the form settings to default and the issue is still there, we should update you here as soon as we get news about it.
