PLEASE HELP ASAP! I'm losing Sales! Nothing on this form is working!

Nothing on this form is working. Not the Next buttons, not even the Coupon Code apply button. Your service is becoming unreliable it seems. 

I was able to fix my form by reverting back to an older version. but there was no reason for this to happen. The only change I made since the version I reverted too, was I created a coupon code today. Your stuff is seriously full of bugs, its ridiculous.
