My form says its disabled

    When I try to fill out this form it says its disabled. Can you please tell me why?

    Your account and forms were suspended because you have form that collect login/username/password of accounts.


    This is a violation to the JotForm Terms of Use.

    We can only request for consideration for the re-activation of the account and forms after you complied and agreed to delete the form/s that were flagged for phishing.

    Let us know if you agree to remove the form/s on your account.

    I legitimately need this information from my clients to build their websites and social media. I am not 'Phishing' which means a fraudulent use of information. When you say remove the forms from my account you want me to delete the WHOLE FORM or just the fields that  ask for logins etc?

    I have gone through and removed all the fields that were login or password related from all my forms I hope this will be enough to reinstate my forms, as I use them for my business all the time.

    I checked your forms and I was able to confirm that you have removed the login/username/password collection in your form. I have activated your form. You can now access your forms properly. Please do avoid collecting such information to avoid your account to be suspended again.
