Form Not Populating Correctly in IE

    The form looks great in Chrome on all various mobile devices. However, IE is still an issue. The way it's presenting is making it impossible to complete. Please advise.

    We have checked your Form in IE 11 Browser on Windows machine, but not got any issue. Please check the below screenshot:
