You have entered incorrect Paypal account on the Payment payment field. That's the reason why your form is not working when redirected to Paypal. You should enter your Paypal email address on the Paypal Account box if you have a personal Paypal account.
1. Click the Paypal payment field in your form, then click the Magic Wand icon to open the payment wizard.
2. On the payment wizard, enter your Paypal Account/Email.
