Issue with address questionaire

Hello, I have had this issue in the past and we cannot recall how we fixed it before. But for some reason, when clients are filling out the address questionnaire, a pop up says something about "only serving PA" or somethings long those lines. We are a business located in WA State so I don't know why it does this? How do we fix it?

Simply go into the edit mode of your form, and remove the hover text, see this example:
