trouble when attempting to import data

When I go to import.jotform.io and click "allow" I then get this message:

Something went wrong when fetching Forms, please try again

Thank you for contacting us. To assist you better is it possible that you provide as a sample of the file you wish to import into your form. You may send it to our support@jotform.com email. Please provide this thread in the subject area so our team know what they are looking for.

See attached list I’m attempting to import.
I get stuck before this step as indicated in the screen capture below.
