The title of my form makes no sense

I have published a form and cannot find the spot where I can rename it's webpage title - so I am a dance studio and my waiver form says Alliance Ju Jitsu.

That's the page that you need to change, to do it, please click on the Preferences icon, you will find it on the top toolbar: 

Once on the Preferences page click on the Advanced tab and click on the Show More button: 

Once there you can change the form's title:
