Why my new forms are disabled?

    When I create and save a new form and try to preview, I am met with an error that says the form is disabled.

    I have checked your account and I can see there are two forms that have been flagged as phishing forms. 

    Your forms have been cataloged as phishing because you're asking for password fields, this is not allowed at JotForm, this is stated on our terms, please take a look here: http://www.jotform.com/terms/ 

    Here are the forms as well as the guilty fields:

    I had to remove those fields from your forms in order to re-activate your account, please avoid ask for this info, otherwise your account will be suspended again.
