Why is my form disabled

    I have now re-activated your account.

    One of your form (payment) is suspected to be used for phishing. I have removed the suspension after finding no violations in your form.

    Just please keep in mind that asking credit card fields or sensitive login information using regular text box fields is not allowed with JotForm. This violates our terms.
