I got all my Forms Disabled and I don't know why

    all my forms are on weebly and I have attached my teacher evaluation form

    Sorry about that, your account was autosuspended by our anti-phishing filter.

    I took the initiative of reviewing all your forms and confirmed none of them violates our Terms

    I checked your account and I noticed that you've added a password field in your Contact Form: http://www.myjotform.com/form/62305930909558.

    Your account got suspended automatically by our phishing tool. You are not allowed to collect "Passwords" or login credentials here in JotForm,  it is against our Terms of Use.

    I went ahead and remove the password field on your form so that I can re-activate your account.
