    One particular form should be embedded in multiple pages but we should know from which page this particular form is submitted. Is it available in Jotform?

    Yes you can achieve this.

    Try putting a hidden field element in the Form.
    Then on each page where a form will be called, try pre-populating the hidden field with the URL parameter from the page(i.e. title of the page). So, when this form is submitted from this page you will be able to identify which page it was used.
