I checked the pages you mentioned on your post and found out that you're calling the form from another html page that houses the actual form:
1. http://www.silverstarevents.co.za/Silverstar_Events/Contact_Us.html
This page uses an iframe and loads this page http://www.silverstarevents.co.za/Silverstar_Events/Contact_Us_files/widget1_markup.html
2. http://www.silverstarevents.co.za/Silverstar_Events/Feedback.html
This page uses an iframe too and loads this page http://www.silverstarevents.co.za/Silverstar_Events/Feedback_files/widget1_markup.html
If you would try viewing the following pages on your iPhone:

You would notice that they are both mobile-responsive. The fields are not out of proportion compared to viewing them straight from the actual page.
To fix this, please try the following:
1. Re-embed the form using this guide Adding-a-form-to-iWeb
2. Remove the mobile responsive widget you added and enable the mobile responsive option from the Form Designer instead. Here's how:
