The only thing appearing on my form in the email I get is the first and last name.

I have checked your form and it can be viewed with no problem in most popular browsers (Chrome, IE and FF)
I have also viewed your submissions and as you can see on the picture below, the submissions include not only the first and last name fields, but all the fields that were filled.
It might be probably your PDF viewer, so I would suggest to try to install a new version

Did you seriously post my employees information on the site???? seriously? REMOVE IT!
The only thing that is in the email is first and last name. I tried to include a pic but it won't let me paste a .jpg in this thread.

Are you guys even reading what I'm saying?!?!?!?!?!?!?!?!
I'm not talking about a browser issue. I'm talking about the email I get!
THe only thing in the email is the first and last name. All other fields are blank. I work mostly from my phone and shouldn't have to log into my jotform account to see the information.
AND REMOVE THE PIC IN THE PREVIOUS REPLY!!!!!!!!!!!!!!!!!!!!! That's someones personal information!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!

I have removed the image from the post above.
I have also recreated your email notification, can you please resave your form; then try a test submission and confirm that your email notification information is showing as you would expect now?

Sorry for posting this map that is a personal information of someone as indicated. We usually have to try and reproduce the issue our customer are facing as a first step on troubleshooting, a reason why we had to try in making submission of your form.
What we are saying in above replies is that the browser your users are using may be causing this, a reason why we are asking to know browser used. please can you try to submit the form by filing it on different browsers and see the same issue occurs when accessing submitted data on your email.
Please lets know if you managed to do the above stated and share with us the feedback to further assost you.
