    Just wondering if we can Closed down a form because it is a breach of the Copyright Act 1968.

    Based on my observation, there are no strong similarities between those two forms, except that both are application forms for clan membership. Copyright is not an issue here - does the form you reported do any harm to your institution?

    I don't think we can close down a form unless it is being used for phishing, or has content that are plagiarized.
