Visual report only displays first 2 questions.

I am trying to create a visual report, but only the first 2 questions appear on the report.

Please note that when you add a new visual report, by default only first two questions are added in the report. If you want to add another questions, it is possible to achieve this by adding another chart form the header menu. Please check the screenshot below:
