Why is my account suspended?

I have an account, but I don't understand why im suspend it.

I see that your account is suspended due to the form titled "Reorden" which is violating our terms of usage. This form is collecting Credit card information which is not allowed using JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. Please refer to our Terms of usage for more information on phishing. 

Your account can be activated only if you agree to our terms and agree to delete the guilty fields from your JotForm. Please let us know so that we can activate your account allowing you to delete the guilty fields from the form.
