Form not found-disabled

I just submitted this issue not too long ago and discovered we are getting this error due to asking for a username. I have removed the question from the form, but need the account reinstated as soon as possible please.

I  believe it was the username field in conjunction with one of your forms being partially titled "Password Request Form".  Since that form was not against our terms of use, I whitelisted the form and it should not longer cause any trouble.  I activated your account and your form should be usable once again.
