My form says it is not found and is disabled, but it's not.

The reason for the same is because our system had suspended your account. The reason why it did the same is because you were asking for username on your form.
As such I would suggest removing the same from your forms which will re-activate your account automatically.
If you do have any issues even after removing those fields, do let us know.

Well I tried with one of the templates too and it didn't work.
