How can I have the PDF form attached to the email look like the terms and conditions of what is in my form?

Okay, you advised me on how to make the email submission look like the contract by editing the email content which I did to great success.  The only thing I need now is to how to make the PDF attachment look look like the email that is sent back with the terms and conditions in it.

If I understand you correctly, you are referring to the attached PDF in the email notifications. You can enable the Show Headers and Texts option in the Submissions. Please follow the steps below:

    1. In My Forms, select the form and then click the Submissions button.

    2. Click the gear or options icon and then enable the Show Headers and Texts option. Click the options icon again to save the changes.

You can also customize the PDF using the PDF Designer. Here's a guide on how to access the PDF Designer:

    1. In My Forms, select the form and then click the Submissions button.

    2. In the Submissions, click the PDF icon beside the Print button. After that, select the Customize PDF option.

    3. The PDF Designer will open where you can customize the PDF.

    Hope that helps. Thank you.
