I can see my form and edit it but when I try to preview it, it says the form is disabled. When I look to enable the form it says it is ENABLED.

I have reactivated your account but I have deleted the said fields. Please refrain from adding the fields again. If you wish to collect a payment at a later date, you can use our payment integrations and enable authorization.
