My form is disabled when I preview it

I can see my form and edit it but when I try to preview it, it says the form is disabled. When I look to enable the form it says it is ENABLED.

Your forms are asking for credit card information using our plain text fields. Please be informed that we don't allow such forms. We consider this as phishing which is against our Terms of Use (http://www.jotform.com/terms/). 

I have reactivated your account but I have deleted the said fields. Please refrain from adding the fields again. If you wish to collect a payment at a later date, you can use our payment integrations and enable authorization.
