Form not found - form is disabled

    i cant access my form. direct link https://form.jotformeu.com/62423222610340 and https://form.jotformeu.com/62422671272351 

    the web says "form is disable" whereas it was a new form I created and is enabled.

    please help me.

    Your form is asking for login credentials which is why your account got suspended automatically by our phishing tool. You are not allowed to collect username and password here in JotForm. Please check our terms of use.
