Why are my forms showing up as disabled?

My forms are all showing up as "Form not found.   Form disabled"

The reason why this happened, is because you created a form to capture credit card information on plain text fields, and this is prohibited by our Terms.

To prevent further issues, you must use any of our payment integrations instead.
