Form disabled for suspected phishing

    You have closed my site due to phishing.  I had a friend on another computer trying to help me.  I can't figure out how to save my form that I made. LOL

    We do not allow for the collection of credit card data directly through forms.  As per our terms of use:

    Collecting Sensitive Information. You may not use the JotForm to collect certain types of sensitive information, including but not limited to credit card information and any type of login credentials. You may collect some sensitive information such as social security numbers or driver’s license numbers, but you are required to use best security practices of JotForm including SSL and Encrypted Forms features. You are solely responsible for compliance with any data protection and privacy laws and rules applicable to the sensitive information.

    I removed the credit card fields from your form, whitelisted your IP address and enabled your account.  You should be able to access your forms once again.

    If you need to add payment authorization to your forms, please do so using one of our payment processors:

    This will allow you to authorize a specific amount to be collected at a later time and ensure processing of the credit card data is done in a secure manner.
