    is there an option for Password protected forms?  possible Username and password?

    The Form Builder does not allow fields on the form that emulate a login form.

    So using username/login/password fields on the form will caused the form to be blocked/disabled by the Phishing filter in place since its against the Terms of Use.

    You may want to consider using alternative solution that works without the need to use username/password fields on the form.

    For more complex alernative, you can also use encryption on your form.
