Form is Disabled?

Can someone tell me why my form suddenly got disabled?

Is there something on the form that is causing an issue?

I have spent a considerable amount of time writing conditional logic and preparing the form. It is a little frustrating that it worked with no issues during that time but suddenly stopped during testing. 

I need to be able to trust that this form will work for my business or I can not deploy it.

Any help / explanation would be very helpful...
