Form Constantly Disabled

    The form I created, worked once and then has shown as "Form Not Found - This form is disabled" ever since. I have tried changing the status to "enabled" and it still comes back as disabled. I'm guessing the system has locked it thinking its phishing? Screenshots attached.

    It looks your account is auto-suspended by our phishing algoritm. It is not allowed to create login forms or collect sensitive information like Credit card numbers, passwords, etc. through JotForm. Such forms are considered as phishing forms and associated accounts are automatically suspended. And I can see your form has a field with Password / PIN label. Please refer to our terms of usage for more information on phishing forms.
