Shipping address removed from my form

Also, I built a form to send to customers offering a free sample of my product. I added a required space for address and phone number but when I click on my form I realized Jotform removed it. Please Explain

I have inspected your form and mentioned field are not shown on your live form, because they are conditional. However conditional field is deleted from your form and now those conditions are not working.

To resolve this issue you need to delete or disable conditions that are set for a field that is deleted from your form.

On my cloned form I have deleted mentioned conditions and now phone number field and address field are shown.

I'm trying to make sure I understand. I worded something incorrectly and you had to delete it in order for the address field to show up again?
When I clicked your link it looks great except for the bottom I will send a picture to show you. Can this be fixed so it is more uniformed?

Yes, you would need to delete the conditional logic with missing fields in order to show the address field in the form. The address field is tied to that condition, but fields are missing there causing you the issue. 

The cloned version of my colleague has the Jotform Footer so I think you mean that. Well, instead of cloning back the form, you can just edit your form to remove the condition to fix the problem.
