How can I create a form with a template

I am trying to create a form using a Contact Us template. I select the template and click the green next button but nothing happens. I have tried this in Internet Explorer, Chrome and on my Ipad but have the same problem every time.

Please advise why this is happening.

It might be related to some blocked resources. The cloning seems to be fine on our side.

Could you please also try to use the template directly form the templates gallery?
