What plan is best for us - Time Sheet forms?

We're a non-profit church, with about 10 employees who need to submit time sheets / hours worked every pay period. (twice / month.)

We don't need fancy effort reporting/ project allocation, just 15 days in the pay period, time in / time out, and total hours each day.  Then total hours by pay period.

Would this be one form, that can be re-used by changing the date each pay period? With 10 users?

You can do what you need with one form that can be reused and, as you said, just change the date. How many submissions are you expecting to have per month since Free plan is limited to 200 per month?

Yes, it is possible to compute for the total hours using the Calculation Widget. You can check this article for more information regarding adding calculation in your form: How to Perform Calculation in the Form.

You will need to manually create your timesheet form. You can hire a third-party developer that can create the form. You can find some freelancer here: http://www.upwork.com/.
