We created a questionaire maybe 2 years ago and I am still getting random responses but I cant seem to log onto the website

Do you mean you cannot access the account of the owner of the form? The account owner of the form (http://www.jotform.us/form/41414965282154) is username: nsc and is associated to the email: admin@newarksoccerclub.org.
