does $90 come out of my bank account all at once? or does it come out monthly? I would prefer if it came out all at once so that way I would not have to worrie about it for the rest of the year. It would also mean I know I have paid it and I can concentrate on doing my work.

It is all at once since you've subscribed Premium Yearly plan. Therefore, next billing date will be on next year , April 15, 2013.

Just making sure I don't have to pay now however when it comes to April 15, 2013 I pay the $90, as I have upgraded 2 to 3 days ago and I thought Jotform.com would of take my money to let me know I have paid for a full year but at the moment jotform.com has not taken the $90 out of my bank account so at the moment I am confused do I get a year free before I pay or does it take 3 to 5 days to process payment or a bit longer.

Yes, you will pay $90 for the year at the beginning of the subscription period. If you would like to pay monthly, you can instead choose $9.95/month plan. We have such a big discount when you buy yearly because you are pre-paying for the whole year.

No that is ok prefer to pay for a year in one go but still confused.

The yearly premium $90 I have paid jotform.com has not come out my bank account yet as I have said before I have upgraded it being now 2 to 3 days and a bit ago and my money has not been removed yet why is it taking so long to be removed, I want jotform.com to take my money already I have transaction details on my paypal account that I have paid jotform.com but the money has not come out of my bank account yet.
Not trying to be rude or anything I think jotform.com kicks ass (very good)

According to PayPal you have successfully send us $90 on April 15th. Maybe they used your credit card on file? You should probably check it on your PayPal account.
