How can I add a patient name field for every page that is printed?

I would like to have the patient name showing at the top or bottom of every page that is printed. How can I do that?

To be able to achieve this on your form https://www.jotform.us/form/61927738383165 you will have to add Page Break field on it so that the form will be break down into multiple pages.

Now on each pages top and bottom, you can add a calculation field that have the value of the 'patient name' field.

I assume you were referring to printing it using the Print button on the form.
