Why I'm not able to log in to my account?

We had a jotform that was created by a former employee:

    http://form.jotform.us/form/52584675965170

We now receive an error when we try to open this that we have maxed out our allotment?  Is there a way we can reset this, and/or can I be provided with access so I can work with this account?

We were not aware that there was a certain number of jotforms we could use.  Is there a cost to increase our numbers?

I would still need to have access in to this account.  We believe she set this up with one of these emails:

    Ashley@travelbynelson.com or tbn@travelbynelson.com

Thank you for your help on this.

I checked your form and the form is under Guest account and it is over the limit because of payment submissions allowed per month limit. I also checked both of emails provided, there are no accounts created under each of those.

Also, I checked the form you provided, it is marked as phishing form since you are collecting Credit Card information which is not allowed by our Terms of Use.

If you have further questions please let us know.

    Thank you!

If we are over the limit, is there a next step to pay for more uses?  Again, you would have to grant me permission to utilize the site, or we would have to start over.  The only other email that may have been used in the set up for this would have been:  ashley.potter634@gmail.com

I guess I'm confused, as I thought that the purpose of jotform was for a company to have their client fill out their information, including credit card info, and authorize such with their signature?  When we set this up, I posted our in a couple of FB groups to pose the question, what do you do to get this information from your clients?  Several answered with jotform, so that is why we set this up to begin with.

What is the jotform used for, if not for the purpose of collecting this information in writing from a client?

I have checked that guest account and there is not email set up so would not be possible which email address was used to create that form, are you receiving submissions via email from this form?

If so, please provide us the email address where you're receiving these submissions.

Regarding to collect Credit Card info, please do note that JotForm is not intended to collect payment info through plain text fields, we have already some payment integrations that you must use if you need to process payments or get payment info from your clients.

Here is the link to our current payment processing apps: https://apps.jotform.com/category/payment_processing

Here is also a guide that will help you to only get the payment info if you do not want to immediately charge your customer: How-to-Enable-Payment-Authorization

    Hope this helps.
