How do I get the form I selected to appear in My Forms?

Where did you select the form? In the template gallery? If yes, after using the template, you should be redirected to the form builder to edit the form.

I've checked your account and you only have a Contact Form (http://www.jotform.us/form/62584799655174) so I think you've failed to use the form. Please try it again. Would you also mind sharing us the form?
