How to create and add image to jot

I am wondering how to pull multiple images from a PDF file, and put them all into one file to create one larger image to upload into my jot form.

If you want to show images on your form, you need to use an image field, which is found under the "Basic Fields" tab:

Unfortunately, it is only possible to upload one image file. You would need to add another Image field type in order to add a new one.
