Minhas Formas estão sendo bloqueadas?

Gostaria de saber por que meus formulário estão sendo bloqueados?

We do not allow for the collection of usernames and/or passwords through forms.  Your form "Master Divulgador" requests Facebook log in details.  This resulted in our automatic phishing detector suspending your account.

I remove the username and password fields from your form then activated your account.  Your forms should be usable once again.  Please refrain from request such information in any further forms.
