Creating a quiz with conditional output

    Hi,

    I am trying to create a quiz, which is radio buttons (yes/no) on the front end for website visitors. However, when the quiz results are sent to me, I would like a pre-set text description to be included conditional each answer to prefill a document.

    e.g.  
    Question: Do you like oranges?

    Answers: Yes or No (radio buttons)

    What I would see in the output (ideally just the relevant description in bold):

    Yes

    Add oranges to the fruit bowl

    OR

    No

    The customer does not like oranges.

     

    At the moment using the Quiz Form app, I can select if Yes or No is correct, but cannot see how to add the description text, and have no answer being actually right or wrong.
    Is this possible to do?  If so, could you please show me how?

    There is a Converted selector widget field available which may work in your case. The quiz app is not required to get 'converted' results.

    Thank you for your help - this is looking very promising ?
    I have created the attached, but cannot seem to get the radio buttons (or dropdown) to show on the published form. They show fine in the editor.
    Could you please advise how to fix this?

    I did check your form's direct URL and it does show the dropdown question correctly. Please check the screenshot below:
