My account was never reset. Therefore my account has been disabled since August 30th.

Please be noted that all the accounts will be reset automatically on the first day of the following month if they are tagged as overlimit. I have checked your account and see that your JotForm has received about 9900 form views for the current month which is why your account is disabled again. Please check the Form analytics of the form to see the traffic information to the form.
