I recently integrated Salesforce into a JotForm form so that populating Jotform would create a new lead in SalesForce with the provided information.
I then mapped the fields between the forms and was told that the integration was successful.
However, I subsequently made a test submission to verify that all was working and did not see a lead generated in SalesForce.
Is there a important step that I might have missed that would prevent the creation of a lead?

I have cloned your form and integrated it with SalesForce, I can see the info is being sent to the SalesForce account without any problem.

I would suggest you to use the same field type in your SalesForce account and JotForm, in case that you're using custom fields.
I will also suggest you to check the settings for duplicated entries in the SalesFoce integration wizard.
Please try integration your form only with the basic fields, full name, email address and company name, submit your form and let us know the results of your tests.
