I have had the same form for the past couple of years, but for some reason now, once my form has been filled out and submitted, I'm not receiving any emails. Not sure what's up with that.Page URL:

I checked your email activity history and found a series of email failures. But recently (just today) it seems to have been fixed already since they are all now getting sent. With this, I'm assuming that there was a different thread which the problem was taken cared of.
But if you are still having trouble receiving emails, please let us know.
