Why does my formularies have been disabled and what can I do to resolve it?

    Hi,

    I've been using Jotform's forms for almost one year and everything was working perfectly, but today, when I tried to create a new form to collect data for performing a draw I realised that all of them were disable. So I came to ask if I did something wrong or forbidden or if it happened an error in the system and how can I solve it. Thanks.

    I have checked your account and I can see it was disabled because of this form https://form.jotformz.com/52907436116657

    Seems like you're asking for an username there, here is a screenshot of the guilty field:

    I would suggest you to avoid using words such as username, user account or any other that my indicate that you're asking for login credentials. 

    I have re-activated your account now, but please do make the change on your form, otherwise your account will be suspended again because of phishing suspicious.
